I, the undersigned, duly apply for membership in the Lehigh Cavaliers Domino & Community Club. Should my application be approved, I promise to abide by the Rules & Regulations of the Organization. I further pledge to maintain my club membership with timely payment of my monthly dues and to support the club in all its activities.
Initial membership fee $25.00 plus 1st month's dues of $15.00 — TOTAL $40.00
